Having the right documents helps us to understand your needs and plan for appropriate academic accommodations:

  1. ADHD—letter from a psychiatrist or psychologist or medical doctor.
  2. Deaf, deafened or hard of hearing—audiogram, audiologist report or letter from a regulated healthcare professional.
  3. Learning disabilities — should be diagnosed through a psychoeducational assessment. Please submit any documentation you have (IEP, IPRC, psychoeducational assessment, etc.) with the Intake Form. All documentation will be reviewed by an Accessibility Consultant to determine whether or not additional or updated documentation is needed.
  4. Mental Health— a completed Disability Verification Form or a letter from a regulated health professional. Students with a mental health disability are no longer required to provide a DSM diagnosis in order to access accommodations.
  5. Mild Intellectual Disability (MID)/Intellectual Disability (ID) — should be diagnosed through a psychoeducational assessment. Please submit any documentation you have (IEP, IPRC, psychoeducational assessment, etc.) with the Intake Form. All documentation will be reviewed by an Accessibility Consultant to determine whether or not additional or updated documentation is needed.
  6. Mobility, acquired brain injury or medical disabilities— the Disability Verification Form or a letter from a regulated healthcare professional.
  7. Vision—photocopy of your CNIB card or a letter from a regulated healthcare professional.

Your healthcare professional should complete the Medical Verification Form when possible.
