- Book an appointment with your doctor or visit a walk-in clinic available in your area. Bring your program specific Prerequisite Health Form to the appointment and ensure your doctor signs and stamps the health form documents after completing all medical requirements.
- Carefully read the detailed instructions on the Prerequisite Health Form (downloadable files are located on the left navigation bar). All medical costs and doctor’s service fees are the responsibility of the student.
- Some sectors continue to require proof of COVID-19 vaccination records from our students in clinical practice courses. This applies to settings such as hospitals, long-term care facilities, childcare centers, and other health care or community-based settings. These sectors, including learning environments at George Brown College, continue to follow public health, regulatory standards and best practices. For up-to-date information, refer to your program specific Prerequisite Health Form documents.
- Failure to complete and submit your health form documents by the due date to the Paramed Placement Pass portal will disqualify you from participating in clinical, dental, or field placement practice which may result in withdrawal from the program.
Special Note to International students currently residing overseas or abroad:
- The Prerequisite Health Form is a separate medical requirement needed by your academic program to qualify for participation in your clinical, dental or field placement. You may begin completing all medical requirements in your home country to help you save time and money before you arrive in Canada (except the Flu shot which you can get it done here in Nov/Dec).
- Please book an appointment with your family doctor/physician in your home country and bring your program-specific Prerequisite Health Form with you and begin completing the medical requirements. Ensure that your doctor properly completes, signs and stamps the Prerequisite Health Form. Additionally, make sure all supporting medical documents, lab test reports, immunization records are translated into English. Failure to do so will result in you having to cover all the extra medical costs and doctor’s appointment in Canada.
- Failure to complete and submit your health form documents by the due date to the Paramed Placement Pass portal will disqualify you from participating in clinical, dental, or field placement practice which may result in withdrawal from the program and will have a negative impact on your Study and Work Permit.
