ACCIDENT REPORTING

All student, staff, or visitor accidents and injuries must be reported. Please download and complete the SUPERVISOR’S ACCIDENT INVESTIGATION REPORT FORM. The report MUST be completed within 24 HOURS. 

(i) In-Class Student Accident
The Professor/Instructor is responsible for completing and returning the Supervisor’s Accident Investigation Report.

(ii) Out-of-Class Student Accident
The Campus Manager is responsible for completing and returning the Supervisor’s Accident Investigation Report.

(iii) Staff Accident
The staff member’s direct Supervisor/Manager is responsible for completing and returning the Supervisor’s Accident Investigation Report.

(iv) Visitor Accident (includes external contractors)
The Campus Manager is responsible for completing and returning the Supervisor’s Accident Investigation Report.

STUDENTS ON WORK PLACEMENT WHO HAVE A WORK RELATED ILLNESS OR ACCIDENT INCLUDING NEEDLE STICK INJURY

WHMIS TRAINING

WORKPLACE HAZARDOUS MATERIALS INFORMATION SYSTEM (WHMIS) 

R.R.O. 1990, Reg. 860, as amended O. Reg. 36/93 WHMIS training is an annual requirement for employees that work with chemicals. Employees who do not work with chemicals must complete WHMIS training every three (3) years.

WHMIS training is mandatory for all employees and offered through Cornerstone.  In person classes are no longer offered.
