Important information for all
Current (Registered) Students:
Some first year and returning students currently registered in the School of Social and Community Services, School of Dental Health, School of Health & Wellness, School of Health Services Management and School of Nursing Programs, must download and complete their specific program Clinical Pre-placement Health Form which has the applicable medical and non-medical requirements needed, prior to the commencement of the school year.
We urge you to start the Clinical Pre-placement Health Form requirements immediately,since it will take you 6-8 weeks to complete. It is your responsibility to ensure that your doctor or health care provider properly completes, signed and stamped your health form. If you do not have a family doctor, you may visit any Walk-in Clinic available. The cost of all immunizations and non-medical requirements are paid for by the student.
To get started, check out the left side navigation bar and download your specific program health form. Please read the instructions carefully and use the links that will help and guide you to complete all the medical and non-medical requirements on your health form.
Once you completed your health form requirements and have confirmed your registration in the program, you may then create an account and book an appointment with ParaMed, the College’s Occupational Health Nurse provider on their on-line service at http://www.georgebrownhealth.ca. You must bring the originals and photocopies of your completed health form and all the important supporting documents at your scheduled appointment time and they will your confidential information.
After your appointment with ParaMed, please allow 48 hours for them to enter and update your information on their website. To view and download your final health form clearance results, log-on using your Student ID and password on their online service at www.georgebrownhealth.ca
As of September 2009, students can now cancel their own appointment online (with 12 hours notice) at http://www.georgebrownhealth.ca. Failure to provide 12 hours notice will result in a $41.20 dollar fine. ParaMed is located at 480 University Avenue on the 7th floor, Suite 704 (Northwest corner of University and Dundas, beside CIBC Bank), (St. Patrick Subway Station). No walk-in visits allowed.
Failure to have the proper health forms and supporting documents completed and submitted by the specific program due date will result for the student being excluded and ineligible for clinical/field placement and therefore unable to proceed to the next semester.
For students registered in the School of Early Childhood Program, please carefully review the registration package sent to you by your academic department or you may visit their Field Placement pages to obtain more information.
