Health Risk Assessment: NEW Service added to the Interprofessional Learning Clinic

The Health Risk Assessment is a valuable tool that provides you with personalized information about your health risks and strengths.  The on-line questionnaire allows individuals to obtain feedback on their general health risks.

Questions are asked around the following health topics:

  • Nutrition
  • Tobacco use
  • Vehicle safety
  • Physical activity
  • Alcohol use
  • Stress and depression
  • Current health
  • Women’s and men’s health
  • Biometric measures (e.g. weight, height, blood pressure, cholesterol)

The on-line questionnaire is personalized and strictly confidential.  You will receive a printed Health Risk Report at the end with summarized tips and facts.  This report is used for health awareness only and if appropriate, can be shared with your health care provider.

The Health Risk Assessment also includes links to a variety of online libraries, resources, and tools, such as a general health library, men’s and women’s health libraries, a body fat estimator tool, a body mass index (BMI) calculator, and a cost-of-smoking calculator.

What does the Health Risk Assessment involve?

The Health Risk Assessment is quick and easy and involves approximately 10-15 minutes to complete and will involve some computer literacy.

Why should I take the Health Risk Assessment?

  • To increase your health risk awareness and knowledge
  • To learn more information on how to increase your overall health
  • You can give yourself an annual health ‘check up’
  • To identify areas of strength and areas needed for improvement, small changes can make a big difference in your health risk!

How do I take the Health Risk Assessment?

If you are interested in taking the Health Risk Assessment please ask at the ILC front desk for schedule times and dates.  The Health Risk Assessment is available to take on-line in The Health Promotion Hub, B101 and the Fitness Suite, B103.  The cost is $10 and can be paid by cash, credit or debit.

The Health Promotion Hub will update this page regularly for any additions/changes in the future.
