C100 & C118 Second Year Students (September 2010 Intake)

Field Placement Clearance Form Requirements

For students returning to complete the second year of this program in September are mandatory required to renew their vulnerable sector police check and CPR Level-C certificate on a yearly basis.

Please be advised that we are still waiting further instructions from OESC in how our students can acquire a police check application form. We will keep you posted.

If you need to renew either your Standard First Aid or CPR Level-C certificate, you may take these courses at any Workplace Safety and Insurance Board (WSIB) approved First Aid Training establishment available in your area. Please visit their website.


In order to receive your upcoming field placement assignment, you must bring your First year-Field placement Clearance form, your valid vulnerable sector police record check and CPR Level-C certificate on the Orientation day on Wednesday, September 1st, 2010.

If you lost or misplaced your First year-Clearance form or your entering as new second year student, you must download the forms below and have your doctor/physician complete the forms again.

All other information regarding your field placement will be given to you on Orientation day.
